If your sleeping partner snores, it could be more than an annoyance: it could be a sign of sleep apnea. This occurs when air flow into the lungs becomes obstructed in the throat for a few seconds during sleep. The obstruction can take many forms, but a common one arises from the tongue relaxing against the back of the throat, producing snoring sounds as air attempts to pass through this restricted area.

Sleep apnea can cause severe problems: lower daily energy levels and mood from poor sleep; lower oxygen saturation that could affect brain function; and increased risk for cardiovascular disease. So, if you’re awakened by your partner’s snoring (or they’re complaining about yours!), it’s important to have it checked and treated.

This begins with a visit to us for a complete oral examination. Like many dentists, we’re well trained in the anatomy and structures of the mouth, as well as the causes and treatment of sleep apnea. We’ll examine your mouth, take into account any possible symptoms you’re experiencing and, if your suspicions are correct, refer you to a sleep physician to diagnose if you have sleep apnea.

Treatment will depend on its cause and severity. An oral appliance worn during sleep is the recommended first treatment for mild to moderate sleep apnea that involves the tongue as an obstruction. We develop a custom appliance that helps move your tongue away from the back of the throat, reducing both apnea and snoring sounds. For more advanced sleep apnea you could benefit from a Continuous Positive Airway Pressure (CPAP) machine. This device generates continuous air pressure through a mask worn while sleeping that helps keep the airway open.

Of course, there are other causes for obstruction, some of which may require surgical intervention to relieve the problem. Abnormally large tonsils, adenoids or excessive soft tissue can all restrict air flow. Surgically removing or altering these structures could help reduce airway restriction.

Whatever type or degree of sleep apnea you or your partner may have, there are solutions. The right treatment will not only improve overall health, it will help both of you get a better night’s sleep.

Lots of people don’t know that April is National Stress Awareness Month; don’t fret if you’re one of them. For many, stress is already a common feature of everyday life. According to the American Psychological Association, 62% of Americans are stressed at their jobs, and stress has been estimated to cause the loss of some 275 million working days every year.

In addition to its other negative physical and mental consequences, stress can also spell trouble for your oral health. It may lead to the problems of teeth clenching and grinding, which dentists call bruxism. A habitual behavior that can occur in the daytime or at night, bruxism is thought to affect perhaps one in ten adults. While the evidence that stress causes bruxism is not conclusive, there’s a strong case for the linkage.

Bruxism sometimes causes symptoms like headaches, soreness or pain in the jaw muscles or joints, and problems with fully opening the mouth. It can be detected in the dental office by excessive tooth wear, and/or damage to tooth surfaces or dental work. Grinding or tapping noises heard at night may indicate that someone is grinding their teeth while sleeping. In children, nighttime bruxism is common and not necessarily a reason for concern; in adults, it may be more troubling.

So what can you do if you’re experiencing this problem? If you find yourself clenching and grinding during the daytime, simply becoming more aware of the behavior and trying to limit it can help. A bit of clenching during times of stress isn’t abnormal, but excessive grinding may be reason for concern. Many of the same techniques used to relieve stress in other situations—such as taking a step back, talking out your issues, and creating a calmer and more soothing environment—may prove helpful here as well.

Occasionally, prescription drugs may cause bruxism as an unwanted side effect; in this case, a medical professional may recommend changing your medication. The use of stimulants like coffee and mood altering substances like alcohol and illicit drugs have also been associated with teeth grinding—so if you’re having this issue, consider foregoing these substances and making healthier lifestyle choices.

There are also a number of dental treatments that can help protect your teeth from excessive grinding. The most common is an occlusal guard or “night guard.” This is a custom-fabricated appliance made of plastic that fits comfortably over your teeth. Usually worn at night, it keeps your teeth from actually coming into contact with each other and being damaged. Occasionally, additional treatments such as bite adjustment or orthodontics may be recommended to help solve the problem.

While most dental problems are caused by disease or trauma, sometimes the root problem is psychological. Such is the case with bulimia nervosa, an eating disorder that could contribute to dental erosion.

Dental erosion is the loss of mineral structure from tooth enamel caused by elevated levels of acid in the mouth, which can increase the risk for decay and eventual tooth loss. While elevated acid levels are usually related to inadequate oral hygiene or over-consumption of acidic foods and beverages, the practice of self-induced vomiting after food binging by bulimic patients may also cause it. Some of the strong stomach acid brought up by vomiting may remain in the mouth afterward, which can be particularly damaging to tooth enamel.

It’s often possible to detect bulimia-related erosion during dental exams. The bottom teeth are often shielded by the tongue during vomiting, so erosion may be more pronounced on the unshielded upper front teeth. The salivary glands may become enlarged, giving a puffy appearance to the sides of the face below the ears. The back of the mouth can also appear red and swollen from the use of fingers or objects to induce vomiting.

Self-induced vomiting may not be the only cause for dental erosion for bulimics. Because the disorder causes an unhealthy focus on body image, bulimics may become obsessed with oral hygiene and go overboard with brushing and flossing. Aggressive brushing (especially just after throwing up when the tooth enamel may be softened) can also damage enamel and gum tissue.

Treatment must involve both a short — and long-term approach. Besides immediate treatment for dental erosion, a bulimic patient can minimize the effect of acid after vomiting by not brushing immediately but rinsing instead with water, mixed possibly with a little baking soda to help neutralize the acid. In the long-term, though, the eating disorder itself must be addressed. The effects of bulimia are devastating to mental and physical well-being, and no less to dental health. The sooner the disorder can be treated the better the person’s chance of restoring health to their mind, body — and mouth.

For anyone else, having a tooth accidentally knocked out while practicing a dance routine would be a very big deal. But not for Dancing With The Stars contestant Noah Galloway. Galloway, an Iraq War veteran and a double amputee, took a kick to the face from his partner during a recent practice session, which knocked out a front tooth. As his horrified partner looked on, Galloway picked the missing tooth up from the floor, rinsed out his mouth, and quickly assessed his injury. “No big deal,” he told a cameraman capturing the scene.

Of course, not everyone would have the training — or the presence of mind — to do what Galloway did in that situation. But if you’re facing a serious dental trauma, such as a knocked out tooth, minutes count. Would you know what to do under those circumstances? Here’s a basic guide.

If a permanent tooth is completely knocked out of its socket, you need to act quickly. Once the injured person is stable, recover the tooth and gently clean it with water — but avoid grasping it by its roots! Next, if possible, place the tooth back in its socket in the jaw, making sure it is facing the correct way. Hold it in place with a damp cloth or gauze, and rush to the dental office, or to the emergency room if it’s after hours or if there appear to be other injuries.

If it isn’t possible to put the tooth back, you can place it between the cheek and gum, or in a plastic bag with the patient’s saliva, or in the special tooth-preserving liquid found in some first-aid kits. Either way, the sooner medical attention is received, the better the chances that the tooth can be saved.

When a tooth is loosened or displaced but not knocked out, you should receive dental attention within six hours of the accident. In the meantime, you can rinse the mouth with water and take over-the-counter anti-inflammatory medication (such as ibuprofen) to ease pain. A cold pack temporarily applied to the outside of the face can also help relieve discomfort.

When teeth are broken or chipped, you have up to 12 hours to get dental treatment. Follow the guidelines above for pain relief, but don’t forget to come in to the office even if the pain isn’t severe. Of course, if you experience bleeding that can’t be controlled after five minutes, dizziness, loss of consciousness or intense pain, seek emergency medical help right away.

And as for Noah Galloway:  In an interview a few days later, he showed off his new smile, with the temporary bridge his dentist provided… and he even continued to dance with the same partner!

Whatever problems you may have with your teeth and gums, there are effective solutions in modern dentistry. But like other aspects of healthcare, dental treatment can be quite costly. For many it isn't what can be done but what they can afford to have done.

If you too have limited financial means, don't lose hope — there are effective ways to manage your dental care, especially with a little planning ahead.

The most important thing you can do to manage dental costs is to prevent disease through consistent oral hygiene — brushing and flossing — at least once a day. Removing bacterial plaque, a film of leftover food particles that can trigger infection, from your teeth will significantly lower your risk of tooth decay and periodontal (gum) disease, two very common sources of dental care costs.

It's also important that you visit the dentist at least twice a year. Although it's an expense, it's worth budgeting because it could, along with daily hygiene, save you money in the future. During these visits we'll remove plaque and calculus (hardened plaque deposits) from hard to reach places you might have missed. We'll also check for developing problems: the earlier they're detected the less the long-term impact on your finances.

We'll also evaluate your individual risk factors for dental disease. Some, like hereditary factors, we can't control. But others, like diet and lifestyle choices, we can alter to significantly lower your chance of disease.

With this risk factor profile, we can then put together an ongoing treatment strategy. Not only will this help prevent or at least reduce problems with your teeth and gums, it will help reduce costs in the long run.

Unfortunately, even with the best efforts we can't altogether rule out problems. We'll need to treat those that arise, and usually the sooner the better. Even so, we can usually take your financial situation into account, such as a less expensive temporary measure until you can afford a more permanent solution. We also have payment programs that can help you manage costs as well.

The important thing is not to delay regular dental checkups. The sooner you begin quality dental care the less of an impact any problems we find will pose to your dental health and your wallet.
