Although primary (“baby”) teeth have a lifespan of only a few years, they’re still important to a child’s current and future dental health. In the present, they help a child eat, speak and smile properly. They also help create a healthy future as placeholders for developing permanent teeth yet to come in.
If, however, a child loses a primary tooth prematurely due to decay, the corresponding permanent tooth could come in misaligned. That’s why we do what we can to help a decayed primary tooth reach its full lifespan. And there are different ways to do this depending on the type of tooth.
With front teeth, which don’t encounter the same chewing forces as those in the back, we may use a tooth-colored filling. This approach is also preferable for appearance’s sake since front teeth are highly visible when a child speaks or smiles.
Primary molars, on the other hand, need a more robust solution. A filling may not be able to withstand the level of long-term chewing forces that these back teeth normally encounter. And because they’re less visible than front teeth, there’s less concern about aesthetics.
That’s why many pediatric dentists prefer stainless steel crowns for molars. Just like their permanent teeth counterparts, a primary crown fits over and completely covers a tooth. They’re typically pre-formed, coming in different shapes and sizes that can then be customized for the tooth in question. After preparing and removing any decayed material from the tooth, we can usually install the crown in one visit with local anesthesia and a sedative (if the child needs it for anxiety).
While a steel crown isn’t the most attractive restoration, it typically handles the higher chewing forces in the back of the mouth better and longer than a filling. That’s especially critical for primary molars, which are some of the last teeth to fall out (as late as ages 10-12). And besides preserving it as a permanent tooth placeholder, a crown also helps the tooth function effectively in the present.
Regardless of what method we use, though, preserving primary teeth is a primary goal of pediatric dentistry. And with a stainless steel crown, we can keep those important back molars functioning for as long as they’re intended.

Cavities can happen even before a baby has his first piece of candy. This was the difficult lesson actor David Ramsey of the TV shows Arrow and Dexter learned when his son DJ’s teeth were first emerging.
“His first teeth came in weak,” Ramsey recalled in a recent interview. “They had brown spots on them and they were brittle.” Those brown spots, he said, quickly turned into caviÂties. How did this happen?
Ramsey said DJ’s dentist suspected it had to do with the child’s feedings — not what he was being fed but how. DJ was often nursed to sleep, “so there were pools of breast milk that he could go to sleep with in his mouth,” Ramsey explained.
While breastfeeding offers an infant many health benefits, problems can occur when the natural sugars in breast milk are left in contact with teeth for long periods.Â Sugar feeds decay-causing oral bacteria, and these bacteria in turn release tooth-eroding acids. The softer teeth of a young child are particularly vulnerable to these acids; the end result can be tooth decay.
This condition, technically known as “early child caries,” is referred to in laymen’s terms as “baby bottle tooth decay.” However, it can result from nighttime feedings by bottle or breast. The best way to prevent this problem is to avoid nursing babies to sleep at night once they reach the teething stage; a bottle-fed baby should not be allowed to fall asleep with anything but water in their bottle or “sippy cup.”
Here are some other basics of infant dental care that every parent should know:
- Wipe your baby’s newly emerging teeth with a clean, moist washcloth after feedings.
- Brush teeth that have completely grown in with a soft-bristled, child-size toothbrush and a smear of fluoride toothpaste no bigger than a grain of rice.
- Start regular dental checkups by the first birthday.
Fortunately, Ramsey reports that his son is doing very well after an extended period of professional dental treatments and parental vigilance.
“It took a number of months, but his teeth are much, much better,” he said. “Right now we’re still helping him and we’re still really on top of the teeth situation.”

You've probably heard that thumb sucking can be harmful to your child's mouth, but do you know why?
Keep in mind that thumb sucking is completely normal in children up to a certain age. In fact, 95% of babies suck their thumb! This is because it provides them with a sense of security and a way to test and learn about their new world. The American Academy of Pediatric Dentistry recommends that parents and caregivers encourage children to cease this habit by age three.
Many children stop sucking thumbs by themselves between the ages of two and four. However, if you are having issues getting your child to stop after this point, you should inform us at your next appointment. Thumb sucking can actually block your child's front teeth from fully erupting and can also push the teeth forward. The number of hours per day and how much pressure your child applies will affect how far out of position the teeth end up. Excessive thumb sucking can also cause your child's jaw to develop incorrectly. This is why it is so important to stop sucking habits before permanent teeth start to erupt.
There are many creative ways that you can help your child cut back and eventually stop sucking his or her thumb. You might try to implement some behavioral management techniques, such as offering rewards after your child goes a length of time without thumb sucking. If your child is old enough to understand consequences, you can simply try explaining what will happen if he or she keeps up with this habit. When do you think is the earliest age that tooth decay can start? Would you be surprised to learn that the answer is… just two months!
In spite of our best efforts, throughout the world tooth decay remains the most common chronic disease of childhood. And the unfortunate truth is, it’s largely preventable. Tooth decay is caused by harmful bacteria living in the mouth. These bacteria produce acids as a byproduct of feeding on the sugar we consume. The acids attack the hard enamel surfaces of the teeth, eventually making the small holes we know as cavities.
For thousands of years, these bacteria have been thriving in the carbohydrate-rich environment of our mouths. Kids aren’t born with these germs — often, however, they are passed directly from caregiver to child. But there are things you can do to keep from passing the bacteria to your children. For example, don’t share toothbrushes; don’t put items in baby’s mouth after you have licked them or put them in yours; and, if you have untreated dental disease, try to avoid kissing the baby’s lips. (And for goodness sake, don’t pre-chew a baby’s food, no matter what any celebrity may suggest.)
One effective way to control tooth decay is by reducing the amount of sugar in the diet. Sodas and candy aren’t the only culprits — fruit juices are also high in sugar. And remember, it’s not just what your child eats or drinks that matters, but when they consume it. Given time, saliva will neutralize and wash away the acids that bacteria produce. But if kids are constantly taking in sugar, the saliva can’t keep up. So give those little teeth a break — limit sugar to mealtimes, and avoid sweet treats at other times of day.
What other steps can you take to stop tooth decay before it starts? It helps to identify kids who may be more susceptible to dental disease. Given the same diet with the same oral hygiene practices, some children are much more likely than others to develop tooth decay. If these high-risk kids receive preventive treatments — such as fluoride varnishes, help with diet modification, and other measures — early tooth decay can be successfully prevented, and even reversed in some cases.

When it comes to dental health, you might say celebrity interior designer and television host Nate Berkus is lucky: Unlike many TV personalities, he didn't need cosmetic dental work to achieve — or maintain — his superstar smile. How did he manage that? Nate credits the preventive dental treatments he received as a youngster.
“I'm grateful for having been given fluoride treatments and sealants as a child. Healthy habits should start at a young age,” he told an interviewer from Dear Doctor magazine. We couldn't have said it better — but let's take a moment and examine exactly what these treatments do.
Fluoride treatment — that is, the topical (surface) application of a concentrated fluoride gel to a child's teeth — is a procedure that's often recommended by pediatric dentists. Although tooth enamel is among the hardest substances in nature, fluoride has been shown to make it more resistant to tooth decay. And that means fewer cavities! Studies show that even if you brush regularly and live in an area with fluoridated water, your child could still benefit from the powerful protection of fluoride treatments given at the dental office.
Another potent defense against cavities is dental sealants. Despite your child's best efforts with the toothbrush, it's still possible for decay bacteria to remain in the “pits and fissures” of the teethâ??those areas of the molars, for example, which have tiny serrated ridges and valleys where it's easy for bacteria to grow. Dental sealants fill in and protect vulnerable areas from bacterial attack, greatly decreasing the risk that future dental treatment will be required.
