When you come to our office for your first dental visit, please bring:
- A list of all of your current medications, including vitamins or other supplements
- Contact information for your primary caregiver and any medical clearances from your physician for dental treatments
- Your insurance information
New & Current Patients
If you are a new patient, you can complete your forms at the link below and submit them securely online. By sending us your paperwork prior to your visit, you will be able to spend more time with our dentist and team and give Dr. Watson the opportunity to study your medical history. Current patients can access their accounts below as well.
